updates and cleanups

Thanks to the (continued) efforts of Winston Smith, anime.plan9.de now
looks a lot cleaner. He cleaned up and pointed out lots of inconsistencies
and naming issues, so anime.plan9.de should be even more accessible now.

Other recent changes are:

- Boys Be has been replaced by higher-quality-versions
- Kero Kero Chime has been added
- Read or Die TV episodes have been replaced by versions done by KAA,
  also, lots of episodes have been added.
